What to prepare/expect at your appointment
New Patient Registration
We ask all new patients to be prepared to complete patient registration forms online or come in 15 to 20 minutes prior to the appointment time. This is so that there is sufficient time to fill in new patient medical and dental history forms.
Please understand that if for any reason something comes up and you’re not able to make it to your appointment, please let us know at least 48 hours in advance to avoid a charge for the lost appointment time. We charge a minimum of $50 for a 20-minute appointment cancellation that has not been advised 48 hours in advance. If the appointment time set aside for you is longer than 30 minutes we may charge you the cost of the treatment that would have been completed in that appointment.
Repeated failure to attend an appointment without 48 hours notice may result in requiring a non-refundable pre-payment deposit if you wish to make an appointment in the future.
